What is this survey for?

The information from the survey will be used to help the State of Washington better understand changes in Washington communities to help better serve them. It asks for basic information about one randomly selected child per household. It should take about 15-20 minutes to complete.


Why was my child chosen?

To help the Washington State Department of Health understand the experiences of the many different children in Washington, many different individuals have been chosen to represent others like them across the state. If you’ve received a letter in the mail, a postcard, or a phone call, then your voice is the one we’ve been looking for. Your answers will tell about not just your child’s situation, but of those like your child throughout Washington.


Are my answers confidential?

Family and children cannot be identified. All names are removed to keep responses confidential and only summarized results will be shared publicly. Only trained staff have permission to work with confidential information and survey data is saved using secure technology.   


When will I receive my gift card?

Gift cards are sent to respondents via email usually within ten business days after you complete your survey. Look for an email with the subject line “WA Child Wellness Survey sent you a reward – $20” from “WA Child Wellness Survey”. If you haven’t received the digital gift card, or have any questions, please reach out and contact us.


Can I take the survey in-person?

The Child Wellness Survey is conducted by mail and/or phone and does not include any in-person or door-to-door interviews. Your participation is entirely voluntary and anonymous. Please reach out if you have any questions.
